Vision coverage varies depending on employment type. Administrative staff and Faculty have coverage through Anthem Blue Cross and Blue Shield while Classified and Bargaining Unit employees vision coverage is administered by Vision Service Plan (VSP).

Exams:  The plan pays $25 for an exam every 12 months; the plan also pays for one of the following every 24 months for adults and every 12 months for *children:

• Single Vision Lenses: $45
• Bifocals: $55
• Trifocals: $75
• Contact Lens: $45
• Medically Necessary Contact Lens: $150

In order to receive vision benefits simply submit an Anthem Vision Claim Form.

classified & bargaining unit coverage
Exams:  VSP will pay for one exam for an adult every 24 months and every 12 months for a dependent *child.

Glasses: VSP covers the following every 24 months for adults and every 12 months for dependent *children (Reimbursement amount depends on agreement between VSP and provider of services):

• Lenses
• Contact Lens
• Frames

In order to receive your vision benefits, follow the convenient steps below:

1. Call a VSP member doctor for an appointment and identify yourself as a VSP member.  You aren't required to complete any up-front paperwork or obtain a benefit form.
   
2. After you've scheduled your appointment, the VSP member doctor will contact VSP to verify your eligibility and plan coverage.  The doctor will also obtain authorization for services and materials.  If you're not eligible for benefits at that time, the doctor will communicate that information to you.

*VSP covers dependent children over the age of 19 at the adult benefit level.
